This role will support the Executive Director of Bi-borough Adult Social Care and Health to effectively deliver the strategic vision, goals and core values of the service through undertaking the role of a Head of Service for the Community Independence Service (CIS) Reablement and Occupational Therapy, directly managing the team managers across the two Boroughs. (RBKC & WCC)

Through your demonstrable knowledge and application of both the Care Act (2014) and commitment to Integrated working with health, you will be expected to;

Demonstrate your passion for promoting people's individuality, diversity and rights to:

Provide an effective and efficient service to vulnerable adults with identified rehabilitation and care needs by working with individuals and their Carers, within their Community. Supporting a service to enhance and promote independent living in local communities with an enabling personal centred approach.
You will be committed to timely and safe discharges by managing a responsive, safe , caring, effective and an efficient service.
Operational oversight of the external Complex OT service provision in WCC in conjunction with Contracts and commission colleagues,
Make sure that services offered are responsive to the needs of all users, including their religious, cultural and linguistic needs, and the needs of users with disabilities.
Manage CIS budget effectively and efficiently ensuring that the Council gets value for money at all times balanced with excellent service delivery
? Directly manage and support the CIS Reablement Team and OT managers across Westminster and RBKC and support to the CIS reablement Multi-disciplinary teams.
Use your practical ability to apply and effectively use theoretical frameworks, Social Care Policies, Rpyal College of OT guidelines and research evidence you will:
Undertake care co-ordinator responsibilities within the Care Programme Approach and within a multi-disciplinary framework.
Oversee reviews and attend multi-disciplinary meetings, to identify and recommend the most appropriate packages of care available.
Ensure that appropriate plans for hospital discharge to the community are made and followed through.
Identify any potential needs/vulnerabilities of Service Users children and work in partnership with Children's Services as appropriate.
Identify the most appropriate carers for Service Users and offer statutory Carers Assessments and Care Plans.
Support pathway and process development/implementation and review to provide a personalised streamlined safe and effective CIS service delivery
Representing the CIS service and ASC at strategic multi agency meetings and forums
Keeping abreast with latest trends and developments in intermediate care and preventative services and apply to service with a view to continuous service improvement
Work collaboratively with internal and external stakeholders, demonstrating the expected leadership values to:

Be actively involved in the provision of services to vulnerable adults with complex support needs and their Carers.
Fully participate in the assessment or care management service.
Promote the development of an effective and efficient service ensure an integrated response to service user needs.
Determine an accurate and sensitive assessment of service user needs.
Identify appropriate resources within the Council, the Health Trust, the local community, non-statutory organisations and primary care to ensure the Service User receives the most appropriate packages of care available.
Represent the service at collaborative, integrated Health and ASC platforms
Establish and enhance key partner relastionships with CCGs, Acute hospital trusts, third sector organisations to ensure effective and timely service deliveryAct in accordance with both National and Local Safeguarding Adult and Procedures to:

Ensure that actions are recorded and adhered to with Support Plans put in place to limit the risks for Service Users, Carers and other relevant parties.
Carry out carers' assessments where appropriate.
Follow through applications for respite and residential care ensuring the full involvement of individual users in these plans.
Provide information, advice, support and guidance to service and carers to enable vulnerable adults to improve outcomes through informed choice.
Accurate and up to date assessments are made of service users' situations.
Review Carers Care Plans in line with the Care Act and statutory requirements.
Be competent in all areas of the Skills and Competency Framework for social workers / OTs and adhere/uphold the codes of conduct laid out by HCPC.
Undertake specific risk management responsibilities in respect of service user or organisational risk.
Through your well-developed and effective communication skills you will:

Present complex/sensitive information in an understandable way, to a range of audiences.
Provide a courteous, helpful and polite response to service users, ensuring that individual needs are recognised and provided for as appropriate.
Effectively liaise and work with other council departments, Primary Care and with other parts of the mental health services in the private and voluntary sector.
Ensure all documentation is recorded and distributed to any relevant parties in line with GDPR legislation.
Adhere to all administrative procedures, making sure that accurate initial assessments are made of service users' situations.
Utilise all IT systems available to maintain a high standard, of up to date record keeping, in accordance with Trust and Local Authority policy and procedures.
* Such other duties, within the competence of the post holder, which may reasonably be required from time to time.

* Work evenings, weekends and occasional public holidays, to meet service requirements where necessary.

Qualifications

* Have a valid professional registration with the Health and Care Professionals Council and adhere to relevant code of Professional Conduct
* Degree in Social Work, Dip SW, CQSW, Or equivalent allied health professional qualification
* Recognised senior management training in leadership, policy or strategy

Continuous Professional Development:
* Take responsibility for your ongoing development, including attending Statutory and Mandatory courses to meet the objectives set in your personal development plan.
* To participate in council appraisal process, setting personal objectives with line manager in keeping with service delivery and developmental requirements.
* To ensure that all staff within the CIS reablement and OT teams are up to date with their required statutory and mandatory training and that appraisals for these staff are appropriately scheduled and completed with respective line managers in keeping with council standards.

Experience You will have demonstrable knowledge experience and understanding of:

* Significant management experience in a similar Health or Social Care setting
* Extensive experience of working within ASC and/ or Health Care Services
* Experience of working within integrated multi disciplinary services, carers, families and other professionals
* Experience of policy development, strategic planning, development of governance systems and change management programmes. Successful multiagency & partnership working
* Robust understanding of 'whole system ' working
* The, Care Act 2014, the Care Programme Approach, the NHS LT Plan, and understanding of Health & Social Care integration and direction of travel
* Service delivery and provision that is culturally appropriate and responsive to the needs of service users
