Role Purpose

Provide leadership to the Adult Social Care Operations Divisions to ensure that people receiving care and support receive timely, high quality and safe interventions that build on individuals strengths, maximise independence and supports their wellbeing.
Contribute to the development of Hammersmith & Fulham's Strategic plans and deploy our available resources and capacity to deliver effective place-based locality working.
Create an environment of collaboration and engagement with our partners and communities to develop asset based approached to improve outcomes for people and place.
Deliver professional leadership for the staff teams directly responsible for and ensure that our practice and approach comply with current legislation and guidance
Work alongside the PSW, OT and Mental Health Lead to ensure that here is a positive culture, which promotes the highest standards of practiceRole Duties

Lead and manage the effective delivery of a strength-based approach to adult social care that promotes early intervention prevention and wellbeing.
Ensure that people who are in need of adult social care and support are able to take positive risks, lead the lives they want for themselves and are able to feel safe.
Ensure that appropriate strategies, policies and procedures are in place to effectively manage and develop the relevant services.
Working alongside Commissioning colleagues to create an environment of collaboration and engagement with our partners and communities to develop asset-based approaches to improve outcomes for people and place.
Deliver professional leadership for the staff teams directly responsible for and ensure that our practice and approach comply with current legislation and guidance and delivers personalisation.
Provide leadership capacity to secure and sustain the necessary changes to culture and practice so that services improve outcomes for people and deliver on our place-based approach.
Effectively manage the workforce resource ensuring that all employees operate within a productive environment and learning culture which is both conducive to the continuous improvement and performance of the service area.
Ensure the voice of the person is heard in everything we do and that people using adult social care are able to influence and shape our future direction.
Contribute to regional developments and ensure we learn and adopt best practice from other areas to support our ambition for continuous improvement.
Be a core member of the Directorate Leadership Team and the Extended Leadership Team of the Council and contribute to the delivery of the vison for Adult Social care
Lead and co-ordinate all business processes across the Directorate, including liaison with corporate colleagues as well as demonstrate personal commitment to customer care by ensuring regular communication, visibility and feedback to service users and front-line employees.
Working with Directorate Leadership Team to establish shared responsibility for the corporate management of the Directorate, supporting cross-Council leadership and change.If you are interested in this role please send your updated CV in the first instance
