Adult Social Services is going through an exciting transformation in Sutton, including a transformation LD programme jointly owned with our Health and Community partners. They are looking for an experienced and motivated leader committed to meeting the needs of people with learning disabilities in Sutton. You will ensure that transition and adult social work and a specialist learning disabilities health team, work together to provide a person centered, cost effective and seamless service for people with learning disabilities that promotes independence, choice and control.

You will be able to work across both children's and adult services and with multiple agencies and functions, including Special Educational Needs and Disabilities, health services, contracts and commissioning and with service providers. You will be able to involve, support and work with parents and with people with disabilities. You will be a skilled strategist, able to spot opportunities in the recent Special Educational Needs and Disabilities inspection action plan and in Sutton Council's introduction of outcome based commissioning, to lead social services for people with disabilities in Sutton forwards.

You must have experience of managing registered providers, working with health partners and experience with Learning Disabilities.
This is an important role and you will be part of the adult senior management team and will report to the Assistant Director of Adult Social Services

As Head of Service you will play an important role in a range of core business activity including:

? Contribute to development and Implementation of Sutton's Learning Disability strategy Statutory duties arising from the Care Act, Children Act, Mental Capacity Act, Children and Families Act, Carers Act and CSDPA (as they apply to children), Short Breaks regulations, and Mental Health Act.

? Effective implementation of Adult safeguarding.

? Assessment of need and support planning which meets needs and makes best use of local resources, including NHS continuing care.

? Reducing health inequalities through support to local health services to make reasonable adjustments for people with learning disabilities, including through LeDeR reviews.

? Re-commissioning supported living services for adults.

? Delivering the local service requirements of Transforming Care.

? Joint commissioning of educational placements with education and the local CCG.

? Communication and engagement with parents and carers, children, young people and adults with learning disabilities. ? Complete the transformation of the Adult LD service.

? Deliver the improvement plan for the LD Clinical Health Team
